Clinical Criteria Summary
Prescriber Requirements
- Provider must be a VA dermatologist or other provider locally designated to prescribe ivermectin 1% cream
Patient Diagnosis & Clinical Presentation
- Documented diagnosis of papulopustular rosacea (may be mixed presentation with other rosacea subtype)
Treatment History & Medication Response
- Inadequate response despite appropriate trials of at least two alternative topical treatments alone
- Inadequate response despite one trial of an alternative topical treatment in combination with an oral antibiotic
Exclusion Criteria
- None
Additional Clinical Notes
- Oral antibiotics used for rosacea include doxycycline, minocycline, azithromycin, and clarithromycin
