Clinical Criteria Summary
Exclusion Criteria
- Major surgery within the prior 2 weeks
- Current serious wound that is not healing
- Any grade GI perforation or grade 3-4 fistula
- Arterial thromboembolic event within the previous 6 months
- QTc >480 ms (Monitor ECGs in patients with congenital long QT syndrome, congestive heart failure, bradyarrhythmias, or those who are taking drugs known to prolong the QT interval, including Class Ia and III antiarrhythmics)
- Uncontrolled hypertension
- Untreated CNS metastases
- Pregnancy
- Lactating
Indications/Inclusion Criteria
- Child-Pugh A, Barcelona Clinic Liver Cancer Stage C or B, unresectable hepatocellular carcinoma as first-line treatment
- Endometrial Carcinoma [mismatch repair proficient (pMMR)/microsatellite stable (MSI-S)] in combination with pembrolizumab, following prior systemic therapy and not a candidate for curative surgery or radiation
- Renal Cell Carcinoma, in combination with pembrolizumab, as first-line treatment
- Renal Cell Carcinoma, in combination with everolimus, following prior anti-angiogenic therapy
- Differentiated Thyroid Cancer, locally recurrent or metastatic radioactive iodine-refractory
General Oncology & Performance Requirements
- Care provided by a VA or VA Community Care oncology provider
- Goals of care and role of Palliative Care consult have been discussed and documented
- Eastern Cooperative Oncology Group Performance Status of 0 or 1
Reproductive Health & Counseling Requirements
- For females who can become pregnant: Counseling provided on potential risks vs benefits of treatment and the use of effective contraception during therapy and for 30 days after stopping treatment.
